Position Summary:
The Supervisor manages a team of Care Anywhere (CAW) Clinical Coordinators and coordinates activities in support of the delivery of Care Anywhere services. In this critical position you will monitor scheduling activities, program referrals, post discharge, ER treat and release follow up, and care coordination activities.  The Supervisor will assist the leadership team with mentoring, training and oversight of the performance of assigned employees.

 

Essential to the Supervisor role is contributing to the success of the company. The Supervisor’s actions should be poised toward business growth and comply with Alignment policies. The Supervisor will collaborate with the department Director and upper management team to promote successful growth within the department and throughout the company.

 

This position is responsible for the day to day operational performance of the Care Coordination, CAW team is to ensure timely coordination of services and adherence to regulatory and compliance standards.

 

General Duties/Responsibilities:

(May include but are not limited to)

 

  • Mentor, train, audit, coach and manage team of care coordinators.
  • Create and maintain an environment that inspires and encourages the growth and engagement of team members.
  • Oversee the process, management and tracking of program referrals, enrolled members, ER treat and Release, post discharge referrals, and hospice.
  • Provide guidance to staff or directly manage complicated coordination requests from members, providers or staff.
  • Assist with the development and oversight of CAW CC programs, projects, workflow, standard operating procedures and reports.
  • Create/Reviews workflows, job aid, standard operating procedures, and/or policies and recommend or change as appropriate to ensure timely, efficient, and effective outputs.
  • Collaborate with other supervisors and managers in the department to improve processes and work flow.
  • Assist with assessment activities and other process improvement strategies to enhance the effectiveness of the CAW Care Coordinator department.
  • Act as a resource to the members, providers, and community partners.
  • Communicate effectively, both verbally and in writing, including presentations as required.
  • Establish and maintain effective interpersonal relationships with staff at all levels, providers, other departments or programs.
  • Build team engagement.
  • Communicate with internal and external customers in a professional manner and follows through on commitments.
  • Initiate and follow through on projects.
  • Work well as part of a team to manage/lead multiple projects simultaneously.
  • Think critically to solve problems effectively.
  • Work closely as a liaison between management and the team to ensure that new cases assigned are touched in a timely manner.
  • Function to gather data for analyzing and reporting purpose.
  • Adheres to Confidentiality and HIPAA regulations.
  • Continued support of at least 1 APC/Extender.
  • Other projects and duties as assigned.

 

Supervisory Responsibilities:

Oversees assigned staff. Responsibilities include: recruiting, selecting, orienting, and training employees; assigning workload; planning, monitoring, and appraising job results; and coaching, counseling, and disciplining employees.

 

Minimum Requirements:

  1. Minimum Experience:
    1. 2+ years of care coordination experience; or any combination of education and experience, which would provide an equivalent background.
    2. 1+ year Care Anywhere Clinical Coordinator Experience.
    3. 1 + years supervisory/lead experience in a health care setting.
  2. Education/Licensure:
    1. High School diploma (required).
    2. Associates/Bachelors (preferred).
  1. Other:
    1. Knowledge of Medicare Managed Care Plans
    2. Demonstrated leadership skills, including the ability to motivate, coach and support team members.
    3. Ability to manage projects/initiatives to improve program performance
    4. Ability to organize, prioritize and delegate workflow
    5. Ability to apply program guidelines and follow procedures
