Position Title: Nurse Practitioner/Physician's Assistant

External Description:

As a leader in the care of seniors, we are seeking a Nurse Practitioner or Physician Assistant who enjoys managing complex patients in a collaborative, highly coordinated setting.  The provider will work closely his/her supervising clinician, social workers, case managers, nutritionists, and other members of our health care team.  Clinician will also participate in multidisciplinary conferences that focus on the frailest members.  He/she will have access to advanced technology, including a robust home monitoring system, Epic EMR, and a proprietary decision support tool that synthesizes data from a variety of sources into actionable alerts that drive care.

Our innovative clinical model values clinician time and judgment over more traditional measures of productivity.  Target patient panel will be less than 500.  New appointment times are one hour, with 20-40 minute follow-up visits.  Our average daily census is less than 18 patients per day.  Clinic hours are M-F 8:00-5:00pm.  Phone call rotates between providers, and after-hours work is rare.  He/she will also have the opportunity to visit attributed patients in the hospital, skilled nursing facilities, and occasionally at home.

Salary is highly competitive.  Additional benefits include medical, dental, 401K retirement plan, malpractice, short-term and long-term disability.  PTO and CME stipend are also included.

General Duties/Responsibilities:

(May include but are not limited to)

  1. Provide primary care services to predominately geriatric population
  2. Perform diagnostic and/or therapeutic procedures within his/her level of training and expertise, and as outlined on the practice agreement and written protocol with the supervising physician.
  3. Orders, interprets and evaluates diagnostic tests to identify and assess patients’ clinical problems and health care needs.
  4. Discusses case with physician when appropriate.
  5. Prescribes medication or other forms of treatment as indicated.
  6. Use of Electronic Medical Records required.
  7. Complete coding and documentation in a timely and accurate manner.

 

Supervisory Responsibilities:

None.

 

Minimum Requirements:

 

  1. Minimum Experience:
    1. 1 year related experience with clinical emphasis for Nurse Practitioner or Physician Assistant; or any combination of education and experience, which would provide an equivalent background.
  1. Education/Licensure:
    1. Masters Degree in Nursing or Graduate of an accredited Physician's Assistant program required
    2. Current unrestricted RN license and NP license in applicable state OR PA license in applicable state and current NCCPA certification required.
    3. Prescribing authority/DEA licensure in good standing as required in the state in which you are applying preferred.
  1. Other:
    1. EMR experience is strongly preferred.
    2. Must be flexible with traveling between centers to meet clinical needs.
