Position Title: Home Monitoring Nurse

External Description:

Responsible for managing patients enrolled through Alignment’s home monitoring platforms including but not limited to virtual health, biometrics/care plan management, telemedicine, etc. 

 This position reports directly to clinical operations leadership with clinical oversight provided by physician leadership in a dual reporting role. 

 

General Duties/Responsibilities:

(May include but are not limited to)

  • Monitor clinical alerts, triage patients as needed that are enrolled in home monitoring programs
  • Adhere to established decision tree as approved by clinical leadership and operate within appropriate licensed scope to deliver medical care
  • Provide disease specific education as patient’s medical condition necessitates and that is within scope of license.
  • Assist patients with home devices and work to troubleshoot any technical or clinical needs. 
  • Take initiative to assist patient with any care coordination needs including facilitating appointments with AHC clinicians, PCPs or specialists; educate on benefit options that can help maximize member’s access to care; refer to appropriate AHC programs; and facilitate any member service related needs.
  • Assist in enrolling identified members into home monitoring programs based on clinician referral or other clinical criteria
  • Manage daily workload to prioritize patients that are at highest risk
  • Strong written, verbal and oral communication skills.  Must be able to effectively communicate with clinicians, other clinical teams, PCPs, care givers, etc.
  • Excellent telephonic customer service skills.  Will be required to conduct video-based patient conferences when requested. 
  • Able to navigate multiple medical record systems to gain comprehensive view of patient
  • Able to clearly and appropriately document patient’s medical status within multiple systems
  • Manage dual reporting structures
  • Other duties as assigned by supervisor

Minimum Requirements:

 

  1. Minimum Experience:
    1. RN – minimum 2 years of nursing experience including case management
    2. LVN – minimum 5 years of nursing experience including case management

 

  1. Education/Licensure:
    1. Unrestricted RN or LVN license within a state
    2. Able to obtain licensure in other states where Alignment has patients (this could be provided with company assistance)
  2. Other:
    1. Medicare Knowledge
    2. Strong Microsoft Word skills
    3. Bilingual, Spanish

Work Schedule: Monday-Friday 6:00 am-3:00 pm with NO required on-call

100% office-based in Orange, CA
