As a leader in the care of older adults, we are seeking a licensed Nurse Practitioner or Physician Assistant Extensivist to provide care to our senior patient population across multiple settings. The ideal candidate will evaluate, treat and manage patients primarily in hospital and skilled nursing facilities with some scheduled outpatient clinic responsibilities. Additionally the provider will see post-hospital discharge patients in the skilled nursing (SNF), intermediate care and assisted living facilities, as well as in the home.

Secondary duties will also include new member evaluations and chronic disease management follow-up in the skilled nursing, intermediate care and assisted living facilities, as well as in the home and outpatient clinic setting.

General Duties/Responsibilities:

(May include but not limited to)

  • Evaluate patients in the hospital, communicate with member, family and care team to coordinate care plan. Communicate with Regional Medical Officer (RMO) to assess appropriateness of plan.
  • Evaluate post-discharge patients in SNF, intermediate care and assisted living facilities, as well as in the home and outpatient clinic setting.
  • Participate in daily 1:1 rounds with RMO to discuss active medical issues and plans for all hospital and SNF patients.
  • Participate in daily multi-disciplinary Alignment Healthcare team rounds to discuss active issues, discharge planning and follow-up for all hospital and SNF patients.
  • Coordinate care with hospital and Alignment Healthcare case managers to ensure smooth transition from the hospital to post-acute care settings.
  • Maintain appropriate and timely documentation of all clinical encounters. Inpatient and SNF encounters to be completed within 24 hours, outpatient encounters within 48 hours.
  • On-call coverage of member patients (telephonic) approximately one weekend/month.
  • Participate in quality improvement projects as necessary.
  • Participate in weekly provider and multi-disciplinary team meetings.
  • Participate in monthly clinical education forums.

Minimum Requirements:

  • Minimum Experience: Prior inpatient experience in general adult medicine or medical sub-specialty.
  • Relevant outpatient general medicine experience will be considered in lieu of prior inpatient experience.
  • Education: Current FL license in good standing- Nurse Practitioner or Physician Assistant.
  • Active DEA license.
  • Strong interest in multi-disciplinary care of older adults required. Experience with managed care preferred.
  • Strong communication skills and ability to work collaboratively required.
  • Travel required within Jacksonville County.
